Requirements

Graduate of a NYS approved LPN program.
Current New York State Licensed Practical Nurse Registration.
One (1) year previous medical/surgical experience within the last five years.
The individual must possess good moral character, patience, and compassion for the elderly, chronically ill and disabled.

Nice To Haves

Two (2) years experience in a Home Health Agency (CHHA, LTHHCP, or Hospice) and/or Long Term Care preferred.

Responsibilities

Provides nursing care under the supervision and direction of an RN which includes: Administration of medications and immunizations.
Observe, measure, record and report clinical data relating to patient’s health status.
Perform clinical procedures such as, catheterizations, trach suctioning and dressing changes.
Provide emotional support and health teaching to patients and families.
Communicate with primary CHN/Home Health RN and other professional disciplines, tolerance, progress compliance and deviations from plan of care.
Maintain current, complete and accurate clinical records per Schofield Certified Home Care policy and regulatory guidelines.
Deliver patient care, treatments and medication administration to assigned patients.
Oversee the HHAs and PCAs.
Perform other related duties in accordance with SCHC policies, as required by their manager.
