Licensed Practical Nurse-Patriots Colony-Full Time-Evenings
About The Position
Patriots Colony is seeking Full-time Licensed Practical Nurses (LPNs) for evening shifts (3pm-11pm). The LPN works under the supervision of a Registered Nurse, Nurse Manager, or Nursing Supervisor, and is ultimately responsible to the Director of Nursing or Assistant Director of Nursing and Director of Quality Care Management. The role involves participating in the development, maintenance, or modification of the nursing plan of care, and providing for the emotional, physical comfort, and safety of residents and their families. The LPN also assists in the supervision of auxiliary personnel.
Requirements
- Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) Upon Hire
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days

Responsibilities
- Administers medications and treatments accurately and according to provider's orders.
- Allows resident choices within limits of capability, considering safety and dignity issues, and follows facility policies and procedures regarding resident privacy and dignity.
- Assigns duties to non-professional staff (e.g., CNAs) according to education, training, policy, and resident need to ensure quality resident care.
- Engages team members for optimal performance and addresses performance as needed.
- Contributes to the development, maintenance, or modification of individual service plans.
- Identifies changes in resident condition with proper documentation according to nursing practice standards.
- Demonstrates consistent follow-through for positive resident outcomes.
- Utilizes good judgment and specialized skills.
