About The Position

The Licensed Practical Nurse (LPN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and Assisted Living Facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.

Requirements

• Current Licensed Practical Nurse (LPN) licensure (in good standing) in the state(s) of practice
• One year of experience under license
• Ability to work well within a team environment
• Ability to work independently
• Time management skills
• Ability to learn an electronic medical records system
• Willingness to drive and reliable transportation
• Valid driver’s license
• Auto insurance

• Experience meeting with a variety of healthcare professionals
• Understanding of state and federal rules and regulations

Responsibilities

• Providing care and services for a group of patients
• Visiting patients where they reside (private homes, Skilled Nursing Facilities, and Assisted Living Facilities)

• Collaborating daily with a team of clinicians
• Working closely with patients on a 1-1 level
• Managing visits and documentation
• Attending required meetings
• Responding to urgent needs by prioritizing and triaging multiple concerns
