About The Position
Join The Christ Hospital Home Health Care team and make a meaningful impact by providing skilled nursing care to patients in the comfort of their own homes. As a Home Health LPN, you’ll provide one-on-one patient care, support recovery, educate patients and families, and work alongside a collaborative clinical team focused on positive patient outcomes. Alternate Solutions Health Network collaborates with health systems in joint venture partnerships to create a post-acute care solution to deliver efficient centralized operations that is patient focused. Requirements
- Current Licensed Practical Nurse license in the state of employment
- Minimum of 1 year of LPN experience in an acute care setting required
- Valid driver’s license and active auto insurance
- Strong communication, time management, and documentation skills
- Compassionate, dependable, and patient-focused
Nice To Haves
- 2+ years preferred
- Home health experience preferred
Responsibilities
- Provide skilled nursing care in patients’ homes under physician orders
- Follow care plans and goals created by the RN Case Manager
- Monitor patient conditions and report changes
- Administer medications and treatments as prescribed
- Educate patients and families on disease management and preventative care
- Help reduce re-hospitalizations through patient education and proactive care
- Complete clinical documentation in accordance with agency and Medicare guidelines
- Participate in on-call, case conferences, trainings, and required meetings
