About The Position
Windsor Grove Nursing and Rehabilitation is seeking full-time and part-time Licensed Practical Nurses to provide high-quality care to residents in a skilled nursing and rehabilitation setting. The role includes contributing to individualized treatment plans, providing direct nursing care, documenting medical evaluations, and supporting programs focused on resident safety. LPNs also supervise and support CNAs, including participating in their evaluations, and perform other duties as assigned to support resident care. Requirements
- Active Licensed Practical Nurse (LPN) license in good standing.

Responsibilities
- Assist in developing and implementing individualized treatment plans for residents.
- Supervise and support CNAs, including participating in their evaluations.
- Oversee the Falls Program and Pressure Sore Program to support resident safety and well-being.
- Provide nursing services and direct patient care.
- Conduct and document medical evaluations upon admission and throughout treatment.
- Perform additional duties assigned to support resident care.
