The Clinical Coordination Nurse (LPN) supports the coordination and execution of telehealth consult workflows across neurology and psychiatry service lines. This role is responsible for facilitating intake processes, supporting case prioritization, and managing real-time dispatch coordination to ensure timely and efficient patient care delivery. Working in collaboration with clinical leadership, physicians, and operational teams, the LPN ensures accurate progression of consults from intake through completion. This position requires strong organizational skills, attention to detail, and the ability to operate effectively in a fast-paced, technology-enabled environment. Core responsibilities include clinical workflow coordination, operational monitoring, technology utilization, and support of training and quality processes.

Requirements

  • Active Licensed Practical Nurse (LPN) license (Florida or Compact State)
  • Minimum of two (2) years of practical nursing or related clinical experience
  • Basic proficiency in computer systems and medical documentation tools
  • Strong verbal and written communication skills with the ability to coordinate across multiple stakeholders
  • Ability to prioritize tasks and manage multiple workflows simultaneously
  • Working knowledge of clinical terminology and patient care processes within LPN scope
  • Ability to apply critical thinking and sound judgment within defined protocols
  • Strong attention to detail and organizational skills
  • Ability to remain calm and effective in high-pressure or urgent situations
  • Proficiency in using telehealth systems, EMRs, and communication platforms
  • Ability to work both independently and collaboratively within a team environment
  • High level of accountability, reliability, and responsiveness
  • Demonstrated problem-solving and analytical skills

  • Experience in telehealth, call center operations, or triage support environments preferred
  • Ability to work in a fast-paced, high-volume environment

Responsibilities

  • Monitor consult queues and manage open or pending cases to ensure timely progression
  • Identify cases exceeding response thresholds and initiate appropriate follow-up
  • Coordinate physician assignment and dispatch based on urgency and availability
  • Manage consult tracking tools including STAT and Psych Hold Boards
  • Monitor queue delays and initiate escalation protocols as needed
  • Track follow-up workflows, including imaging and stroke-related processes
  • Collect and document key patient and case information from facilities
  • Support prioritization of consults using established protocols and clinical judgment within scope
  • Facilitate communication between facilities and physicians to ensure readiness for consults
  • Assist in coordination of specialized workflows, including EEG and psychiatric consults
  • Serve as a communication liaison between physicians, facilities, and internal teams
  • Monitor and respond to physician communication channels in real time
  • Escalate clinical, operational, or technical issues to appropriate leadership
  • Manage escalations from providers, facilities, and internal staff
  • Utilize telehealth platforms and internal systems to manage case workflows
  • Monitor system performance including dispatch alerts and escalation triggers
  • Support downtime procedures to maintain continuity of operations
  • Escalate technical issues impacting workflow or physician connectivity
  • Participate in shift huddles and support effective handoff communication
  • Lead huddles in the absence of leadership when needed
  • Assist in onboarding and training of new team members
  • Maintain knowledge of current protocols, procedures, and updates
  • Identify and communicate quality or process concerns to leadership
  • Review internal communications regularly to stay current on updates
  • Attend required meetings and training sessions
  • Perform additional duties as assigned
