LICENSED PRACTICAL NURSE PT
About The Position
Community Health Services is seeking a skilled Licensed Practical Nurse (LPN) for their Fremont, Ohio office, with the flexibility to support other locations as needed. This full-time position offers the opportunity to work alongside a dynamic team of healthcare professionals, utilize your skills, and grow in your profession. Under the direction of the Charge Nurse and Director of Nursing, the LPN will be responsible for patient assessment, point-of-care testing, lab and diagnostic follow-up, and other nursing functions to support quality patient care. The role emphasizes working directly with patients Monday through Friday, with no weekend hours. CHS is a drug-free/nicotine-free organization, and candidates must pass a drug and nicotine screening upon employment offer.
Requirements
- Graduate of a current accredited Nursing Program
- Current License to practice in the State of Ohio as a Licensed Practical Nurse (LPN)
- Current Basic Life Support Certification
- Knowledge of documenting with EHR
- Strong communication skills and the ability to multi-task
- Ability to critically think and problem solve
- Reliable, professional, and compassionate
Nice To Haves
- Minimum of one year as an LPN preferred
Responsibilities
- Perform Adult and Pediatric Nursing Assessments
- Communicate clearly with Providers, Patients, and CHS Staff
- Follow-up on Lab and Diagnostic testing
- Administer injections as needed
- Follow all safety, HIPAA, and regulatory guidelines
