About The Position

Dungarvin is seeking a Licensed Practical Nurse (LPN) to support the health, enrichment, and development of individuals with developmental disabilities in adult residential facilities in Seymour, Indiana. This is a case-management role involving semi-annual physical assessments and coordination of individualized health care management. The schedule is full-time, Monday through Friday, typically 8 a.m. to 5 p.m., with flexible hours and occasional travel. The nurse participates in an on-call rotation for one week per month, limited to triage of medical concerns.

Requirements

• Current Licensed Practical Nurse (LPN) license in the State of Indiana.
• Skilled, flexible, self-motivated, and dependable, with the ability to work cooperatively with staff, interdisciplinary team members, and outside agencies.
• Strong verbal and written communication skills.
• Ability to work independently and efficiently.
• Organizational and computer skills.
• Acceptable background check and driving record check.

Nice To Haves

• One to two years of experience working with individuals with disabilities or in other long-term care.

Responsibilities

• Ensure each individual receives quality, individualized health care management to support optimal health.
• Conduct routine site visits, check vital signs, perform assessments, counsel staff on medication administration, and document progress.
• Serve as a health and medical resource for staff and provide training in medical matters.
• Conduct medication audits and reviews.
• Ensure compliance with state requirements and organizational policies regarding medical care and records.
