About The Position

This is a full-time position for a Licensed Practical Nurse (LPN) in Home Health, located in Sparta, NC. The role involves providing nursing care, participating in patient assessments, documenting observations and responses to treatment, and assisting RNs with care plans. The nurse will implement care plans under supervision, communicate patient status changes, organize workload efficiently, and maintain current nursing practice knowledge. Documentation of assessments, procedures, education, and communication is crucial. The position also includes providing and assessing the effectiveness of patient and family education under the direction of a Registered Nurse.

Requirements

  • Graduate from an accredited School of Practical Nursing.
  • BLS required per policy guidelines.
  • Current LPN license or temporary license as a Licensed Practical Nurse Petitioner in the state in which you work and reside, or meet licensure requirements in your home state if declaring an NLC state, or current LPN license or temporary license in the state where the LPN works for Non-National License Compact states.
  • Must be able to lift and support the weight of 35 pounds.
  • Must speak English in good, understandable terms.
  • Intact sense of sight, hearing, smell, and touch.
  • Finger dexterity.
  • Critical thinking and ability to concentrate.
  • Must be able to respond quickly to changes in patient/resident and/or unit conditions.
  • Physical Abilities Testing required.

  • Additional education, training, certifications, or experience may be required within the department by the nurse leader.

Responsibilities

  • Participate in determining nursing care needs based on patient data.
  • Recognize and determine the need for immediate intervention.
  • Document patient observations, treatments, and responses to treatment.
  • Participate in treatment and assisting with procedures.
  • Participate in discharge planning.
  • Assist RN with development/revision of the plan of care by providing data and identifying potential goals and interventions.
  • Communicate with clinical staff regarding significant changes in patient status.
  • Implement the identified plan under the supervision of the RN, physician, or other independent licensed practitioner.
  • Organize workload and prioritize responsibilities to ensure quality patient care.
  • Attain knowledge and competence reflecting current nursing practice and perform nursing skills within legal boundaries and facility policies.
  • Document assessments, procedures, patient/resident/family education, and communication in a timely manner.
  • Provide and assess effectiveness of patient/resident and family education under the direction of the Registered Nurse.
