Under the direction of a provider or registered nurse, the Licensed Practical Nurse supports providers in the examination and treatment of patients. This includes preparing exam rooms and instrumentation, operating medical equipment, and administering medications and injections as related to patient care requirements. The role involves establishing the reason for the visit, documenting patient complaints, and maintaining detailed records in the EMR system. Key responsibilities include obtaining baseline patient data, ensuring necessary lab work and diagnostics are completed, preparing for physicals and well-child visits, and implementing patient care plans. The LPN will follow up on provider requests, administer and evaluate medications/immunizations, and provide patient education on health, illness, and self-care practices. They will also teach health-related classes, manage patient flow, maintain exam rooms, and order supplies. Proficiency in EMR systems for documentation, order entry, and communication is essential. Collaboration with public health and social service agencies, and serving as a member of a Patient Center Medical Home team are also key aspects of the role. The position requires providing telephone support, evaluating patient issues, and maintaining updated knowledge of telephone care practices and clinical skills.
