About The Position

The Licensed Practical Nurse supports safe, consistent, resident-centered care in a post-acute senior healthcare setting in Meridian, Mississippi. The role provides hands-on nursing services and clear clinical documentation during the 7 a.m.–7 p.m. day shift. Services in the care setting include short-term rehabilitation, transitional care, and Alzheimer’s and dementia care. The nurse organizes care based on shift report and resident priorities, coordinates with CNAs and therapy staff, responds to call lights and new symptoms, and communicates professionally with families. The role includes reassessing residents, keeping documentation current, communicating clinical updates to appropriate leaders or providers, and preparing a complete handoff to the next team.

Requirements

Active Mississippi LPN license in good standing.
Strong attention to detail, sound judgment, and compassionate communication.

Skilled nursing or rehabilitation experience.

Responsibilities

Administer treatments and medications as ordered.
Complete and update admission and ongoing assessments.
Contribute to individualized resident care plans.
Supervise and support CNAs, including providing input to performance evaluations.
Monitor and document fall-prevention initiatives.
Monitor and document skin-integrity and pressure-injury initiatives.
Promptly escalate changes in residents’ conditions.
Coordinate care with CNAs and therapy staff.
Respond to call lights and new symptoms, reassess residents, and communicate updates to appropriate clinical leaders or providers.
Answer family questions professionally.
Keep clinical documentation current and ensure orders are addressed, notes are complete, and handoff information is clear and accurate.
