About The Position

Responsible for providing direct nursing care to Residents. Participates as a member of the interdisciplinary team providing support and information about nursing services. This role involves ensuring resident safety, administering medications, documenting care, supervising CNAs, and maintaining confidentiality. The position also requires assisting in care plan development, employee orientation, and maintaining positive relationships with healthcare team members, residents, and visitors. Ancillary duties include assisting residents with personal care, following safety procedures, and pursuing professional growth.

Requirements

Unencumbered, active license as a Licensed Practical Nurse in the state of permanent residence (must be from a state within the Nurse Licensure Compact).
Sincere desire to work with elders and others who have a limited capacity for self-care.

Responsibilities

Assure Resident safety at all times.
Prepare and administer medications as ordered by the physician, review medication records for completeness, accuracy and adherence.
Complete required documentation in an accurate and timely manner for each problem and/or need identified.
Supervise care delivered by Certified Nursing Assistants.
Adhere to Resident abuse policy and procedure.
Maintain confidentiality of all activities, persons, and records.
Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain current clinical status.
Attend and participate in departmental/facility meetings as required.
Consistently demonstrate patience and encouragement in dealing with Residents who are limited in their ability to consume medications.
Identify Resident by name-band or photograph prior to administering meds.
Resident preferences are followed when providing care and service.
Maintain Resident privacy, values and right for decision making.
Immediately notify supervisor and Executive Director for any injury, illness, or if develop a condition, that might impact the aptitude to perform job functions.
Perform duties in compliance with established policies and procedures.
Assist in the development and implementation of an individualized plan of care for each Resident.
Assist in ensuring effective orientation of all new employees in accordance with the orientation manual.
Demonstrate compliance with Residents’ Rights at all times.
Establish and maintain positive rapport with all members of the healthcare team, Residents and all visitors.
Help Residents with bathing, dressing, personal hygiene, moving in bed and standing and walking.
Follows policy and procedure during fire/disaster drills.
Establish objectives and goals for self.
Achieve a minimum of two (2) objectives in twelve months.
Strive to promote own growth in health care field by demonstrating knowledge gained by reading literature, attending workshops and in-service education programs, etc.
Legally and safely operate automotive equipment.
Adhere to and comply with any applicable facility insurance guidelines to enable coverage under the policy.
Accept additional assignments within the scope of training, education, or experience as assigned by Charge Nurse or Executive Director.
Maintain the confidentiality of all Resident care information including protected health information.
Report known or suspected incidents of unauthorized disclosure of such information.
Ensure that the Resident’s rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints, are well established and maintained at all times.
Review Resident complaints and grievances and make written reports of action taken.
Discuss such actions with Resident and family as appropriate.
Ensure that you and all facility personnel, Residents, visitors, etc., follow established safety regulations, to include fire protection/prevention, smoking regulations, infection control, etc.
Ensure that the building and grounds are maintained in a clean, safe, and sanitary manner as well as in good working order by reporting all concerns to management.
Ensure that you and all facility personnel follow established regulations governing the use of labels and SDSs for hazardous chemicals.
Ensure that facility procedure manuals identify safety precautions and equipment to use when performing tasks that could cause bodily injury or exposure to a hazardous chemical or infectious waste.
Ensure that you and all facility personnel follow established ergonomics policies and procedures to prevent self-injury.
Ensure that you and all facility personnel performing tasks that involve potential exposure to blood, body fluids, or hazardous chemicals are appropriately trained prior to performing such tasks.
