Weekend Home Health LPN - Licensed Practical Nurse
About The Position
Beacon Home Health is seeking a Licensed Practical Nurse (LPN) to provide quality patient care and skilled services in patients’ homes in Cleveland, Tennessee, and surrounding communities. The LPN works within the scope of practice established by state nursing laws, follows the plan of care, and coordinates care with other members of the healthcare team. The role involves educating patients’ families, maintaining a healthy patient environment, and observing, documenting, and reporting patient responses and changes in condition. The LPN collaborates with physicians and registered nurses, documents care promptly and accurately, and does not alter the plan of care. This is a PRN weekend position with travel to multiple patient residences in the assigned area.
Requirements
- One year of clinical LPN experience.
- Current LPN license in the state where services are rendered.
- Current CPR certification.
- Valid driver’s license and vehicle insurance.
- Access to a dependable vehicle or public transportation.
- Ability to reliably commute to multiple patient residences daily within the assigned geographic area.
Responsibilities
- Provide skilled patient care in the patient’s home in accordance with the plan of care and state nursing practice laws.
- Coordinate care with other members of the healthcare team.
- Instruct the patient’s family in the care of the patient and maintain a healthy environment for the patient.
- Observe, record, and report the patient’s response to treatment and changes in condition daily to the supervising nurse and/or physician.
- Collaborate with physicians and registered nurses.
- Document care accurately and promptly.
- Do not alter the plan of care.
- Commute reliably to multiple patient residences daily within the assigned geographic area.
