About The Position
Westfield Family Physician's PC is an independent primary care practice serving Western New York communities. The practice is recognized by the NCQA as a Patient Centered Medical Home and provides comprehensive, accessible, coordinated, patient-centered care. It offers a supportive work environment with a modern office setting, an integrated EMR, and team-based management. The practice is seeking an LPN who is passionate about caring for patients across the lifespan and enjoys partnering with patients, providers, and specialists. The role serves two offices, in Westfield and Sherman, New York, and requires travel between them. The schedule is four 10-hour workdays, with occasional Saturday mornings required. New graduates may be considered, and the practice provides orientation and training.
Requirements
- Licensed Practical Nurse (LPN).
- Ability to travel between the practice's Westfield and Sherman, New York offices.
- Occasional Saturday morning work is required.
Nice To Haves
- Prior nursing experience is preferred, but new graduates are considered.
Responsibilities
- Room patients and take vital signs.
- Perform simple in-office laboratory tests.
- Answer the phone.
- Administer immunizations and other injections.
- Educate patients and help them navigate the health care system.
- Partner with patients, providers, and specialists to provide high-quality, patient-centered care.
- Travel to either the Westfield or Sherman office as needed.
