The LPN / RN provides direct primary nursing care to residents and supervises the care provided by certified nursing assistants. This role involves keeping administrative nursing staff informed on personnel development, providing feedback to Nurse Managers regarding the performance of nursing assistants, and participating in committees as assigned. The position requires the ability to address service concerns and follow up in a timely manner for resolution, supporting the facility’s Service Excellence Initiative. The LPN / RN effectively supervises and directs licensed and non-licensed nursing personnel, and is responsible for developing CNA's resident care assignments and other duties as needed prior to the beginning of each shift. This role promotes and maintains positive and collaborative working relationships with physicians, physician-extenders, lab, diagnostic personnel, and pharmacy to ensure proper and safe care of residents, as well as with nursing personnel, other departments, residents, families, and visitors. Key responsibilities include observing residents daily, reporting significant changes in resident condition to the physician, completing routine nursing documentation accurately, concisely, and timely, and administering medications using the five rights of medication administration while adhering to infection prevention and control practices. The LPN / RN provides and/or receives shift reports, implements pharmacy processes, performs physical assessments, recognizes normal and abnormal physiological findings, performs wound care, assists in the development and updating of the residents’ plan of care, and communicates effectively with family members regarding resident care and condition. The role also includes completing accident/incident reports, notifying physicians and families of accidents/incidents, performing routine nursing skills such as phlebotomy, IV therapy, wound care, medication administration, catheter insertion, tube feeding management, oxygen administration, tracheostomy care, airway management, and physical/skin assessments. The LPN / RN communicates staffing problems and needs, reports service and unit concerns, receives and provides end-of-shift reports, assists with resident admissions, transfers, and discharges, reports maintenance and housekeeping needs, and maintains accurate narcotic records, validating narcotic/scheduled medications before and after shifts, and notifying the unit manager of discrepancies.
