About The Position

Administer practical nursing care to patients in accordance with physician’s orders under the direction and supervision of a Registered Nurse. This job description reflects management’s assignment of essential functions; and nothing in this herein restricts management’s right to assign or reassign duties and responsibilities to this job at any time.

Requirements

• Successful completion of an accredited program of practical nursing.
• Unencumbered/unexpired LPN license issued from NYS Department of Education.
• Experience with Electronic Medical Record technology and MS Office (Outlook etc) required.

Nice To Haves

• Experience in home health care preferred.

Responsibilities

• Provide health education according to the teaching plan established by the RN.
• Provide routine LPN nursing care as outlined in the nursing care plan and within the scope of practice for an LPN.
• Observe signs and symptoms, and report patient’s reactions to treatments including drugs and changes in the patient’s physical or emotional condition, to the physician and RN Case Manager.
• Document any services rendered to the patient, changes noted in the patient’s condition, and/or family and home situations.
• Document communication with RN Case Manager and physician.
• Other duties as assigned.
