Requirements

  • License to practice nursing as a licensed practical nurse issued by Ohio Board of Nursing pursuant to Section 4723 Ohio Revised Code
  • MDS certification issued by ANNAC (i.e. American Association of Nurse Assessment Coordination).

  • The final candidate selected for the position will be required to undergo a criminal background check.
  • The final applicant selected for this position will be required to submit to a urinalysis drug screen for illegal drug use prior to the appointment.

  • Under the direction of the RN Supervisor, help with one‑on‑one staffing and provide hospital coverage for individuals as directed by RN Supervisor.

  • Under the direction of the RN gather necessary information on quarterly physical inspections and Moses Scales for assigned individuals.

Responsibilities

  • Works under direction of Registered Nurse (RN) and requires substantial knowledge of skilled licensed practical nursing procedures in order to provide care for residents.
  • Administer medication and perform treatments prescribed by the Physician.
  • Administer controlled narcotics and non‑controlled medications in oral, rectal or injectable form (i.e. intramuscular and subcutaneous injections only).
  • Do med awareness and self med programs as written.
  • Provide instructions to families for pass meds.
  • Maintain supplies in medication rooms and treatment rooms of assigned areas and re‑order meds as needed.
  • Perform skilled practical nursing procedures or treatments, (i.e. sterile dressings, administer oxygen, insert catheters, reinsert gastrostomy tubes, suctioning, irrigations, give enemas, douches collect specimens for diagnostic purposes, etc.).
  • Monitor environment in the areas of health and safety, reports and/or remediate problems as appropriate.
  • Assist in emergencies (i.e. first aide, CPR, psychotropic emergencies, etc.)
  • Access emergency box for necessary medications/narcotics, check in medications from pharmacy deliveries and maintain proper record keeping.
  • Deliver medication to appropriate cottage med carts.
  • Participate in change of shift report and check individuals reported as being ill or injured and document appropriately.
  • Make rounds in assigned areas and observe and document resident response to treatment and other significant activities.
  • Observe, document and report individual’s conditions (i.e. medication reactions, treatment responses, individual behavior and progress) and/or maintain related reports and records (i.e. unusual incident reports, patient records, medication reports, restraint records and logs).
  • Provide general nursing care, direct care and/or programming assistance in areas of personal hygiene, grooming, eating, toileting, mobility and behavior, etc.
  • Transcribe and implement physician's orders and start AIR forms as appropriate.
  • Attend in-services, workshops, meetings and seminars.
  • Assume responsibility for own professional conduct and competence.
  • Contacts family/guardians with necessary medical information.
  • Coordinate with Residential Care Supervisor and RN in providing health care to residents.
  • Provide input into Health Assessment and Nursing Care Plan of residents within assigned areas.
  • Coordinate and gather information into IPPs under RN direction.
  • Assist RN and Residential Care Supervisor in orientation of new employees as appropriate.
  • Communicate significant information verbally and in writing to immediate supervisor and other team members as directed by SODC Directive and Nursing Policy and Procedure.
  • Complete paperwork on scheduled primary assignment (i.e. quarterly summaries, IPP's, medical appointments, PO sheets, MARS, chart audits, etc.)
  • Complete required immunizations/PPDs on assigned individuals.
  • Communicate medical information to parents/guardians when appropriate.
  • Monitor healthcare charts on cottage to ensure proper follow-up continuity of health care.
  • Verify monthly physician’s orders and MARs.
  • Change over monthly POs and MARs on last day of month as assigned.
