About The Position

ResCare Community Living is now part of Sevita. This role involves planning and managing patient care according to each patient's needs. The nurse will interview patients, record their medical history and physical condition, obtain vital signs, and provide routine care. The company has five decades of experience in the disability services field, providing support to individuals who need assistance with daily living due to an intellectual, developmental, or cognitive disability. They offer a comprehensive range of high-quality services, including community living, adult host homes, behavioral/mental health support, in-home pharmacy solutions, telecare and remote support, supported employment and training programs, and day programs.

Requirements

Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse
Knowledge of individual service plans, including understanding of client rights and health needs
Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice
CPR Certified
Excellent interpersonal skills, including customer service, patience, empathy, and compassion
In-depth understanding of patient care policies and regulations
Familiarity with symptoms and signs of improvement for several medical conditions
Experienced Nurse required, high medical home.

Responsibilities

Provide 1:1 care for client always following client’s plan of care while maintaining client's dignity and independence
Monitoring clients’ vital statistics and reporting abnormal findings to the case manager and physician assessments
Performs medication administration, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
Complete routines as established by other care professionals such as Physical Therapist, Occupational Therapist, and Speech Therapist
Ensures infection control policies are maintained at all times, reports possible communicable diseases or infections, and ensures the overall implementation and practice of infection control procedures are implemented
Accurately documents nursing actions of all care given and communicates with family, case manager, and physicians as well as documents the progress and outcomes for established goals and informs physician, case manager, and families of changes in client's medical condition and needs
Orders supplies and medications, as needed, to ensure adequate amounts are on hand at all times
Ensures all patient needs are being met in accordance with the instructions of the physician.
Monitors incidents involving individuals. Ensures incident reports are completed timely, accurately, and case managers are informed; examines the individual following the incident
Ensures all medical equipment is maintained and reports faulty equipment to Durable Medical Equipment for repair
