About The Position

Slovene Home is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our skilled nursing team. The LPN will provide high-quality nursing care to our residents while working collaboratively with physicians, registered nurses, nursing assistants, families, and other members of the interdisciplinary care team. The ideal candidate is committed to resident-centered care, demonstrates strong clinical judgment, and takes pride in creating a safe, respectful, and supportive environment for our residents.

Requirements

  • Current and unrestricted Licensed Practical Nurse (LPN/LVN) license in the state of Ohio.
  • Graduate of an accredited practical nursing program.
  • Current CPR/BLS certification, or ability to obtain certification as required.
  • Strong communication, organizational, and clinical skills.
  • Ability to work effectively as part of a multidisciplinary team.
  • Compassionate, dependable, and professional approach to resident care.

Nice To Haves

  • Previous experience in skilled nursing, long-term care, rehabilitation, or geriatric nursing is preferred.

Responsibilities

  • Provide direct nursing care to residents in accordance with physician orders, care plans, and facility policies.
  • Administer medications and treatments accurately and safely.
  • Monitor residents' conditions and promptly report changes to the appropriate nurse or healthcare provider.
  • Complete accurate and timely nursing documentation and maintain resident records.
  • Perform nursing assessments and contribute to individualized care planning.
  • Assist with wound care, vital signs, treatments, and other clinical procedures within the scope of practice.
  • Communicate effectively with residents, families, physicians, and the interdisciplinary care team.
  • Supervise and provide direction to nursing assistants and other assigned staff.
  • Respond appropriately to emergencies and changes in resident condition.
  • Promote resident dignity, comfort, independence, and quality of life.
  • Follow infection-control procedures, safety standards, and all applicable federal, state, and facility regulations.
  • Participate in quality-assurance and performance-improvement initiatives as assigned.
