About The Position

Adoration Home Health is seeking a dedicated Home Health Licensed Practical Nurse to provide patient-centered care in Sikeston, Missouri, serving Scott County. The role is full-time and involves home health visits. The nurse provides one-to-one care in accordance with each client's plan of care, supporting the client's dignity and independence while working independently with team support.

Requirements

• LPN/LPT/LVN degree from an accredited college of nursing.
• Current unrestricted registration and license in the state of practice and state of residence.
• Valid driver's license, acceptable driving record, and proof of car insurance.
• Professional CPR certification.

Nice To Haves

• At least one year of nursing experience.
• Home health care experience.

Responsibilities

• Provide one-to-one care according to the client's plan of care while maintaining the client's dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as needed under the client's plan of care, and assess the results.
• Accurately document care provided, nursing actions, communications, progress, and outcomes toward established goals.
• Communicate changes in the client's medical condition and needs to the physician, case manager, and family.
• Coordinate or order supplies, medications, and feedings to ensure adequate quantities are available.
• Ensure patient needs are met in accordance with physician instructions.
