About The Position

TRU Healthcare is seeking a Licensed Practical Nurse (LPN) to provide compassionate, skilled nursing care to patients in their homes. The role supports patients in remaining safe, comfortable, and independent at home while delivering care in accordance with established care plans and physician orders. Full-time and part-time salaried opportunities are available, with work arrangements intended to align with availability. The position serves patients throughout the Twin Cities area and includes travel between patient visits; mileage reimbursement is provided.

Requirements

Current, valid Minnesota LPN license.
CPR/BLS certification.
Strong clinical, communication, and organizational skills.
Ability to work independently while collaborating effectively with the care team.
Ability to travel throughout the Twin Cities area.
Reliable transportation and willingness to travel between patient visits.
A genuine desire to make a difference in the lives of patients and their families.

Previous home health experience preferred.

Responsibilities

Provide skilled nursing services to patients in their homes according to established care plans and physician orders.
Monitor patient conditions, identify changes in health status, and communicate concerns to appropriate care team members.
Administer ordered treatments within the scope of practice.
Perform nursing assessments and provide interventions based on patient needs.
Educate patients and caregivers about medications, treatments, disease management, safety, and other aspects of care.
Accurately and promptly document patient care and communicate with the interdisciplinary team.
Build relationships with patients and families while providing compassionate, patient-centered care.
Collaborate with the interdisciplinary team to support continuity of care and positive patient outcomes.
