About The Position
Affirmed Home Care is seeking compassionate, skilled Licensed Practical Nurses (LPNs) to join their home care team. This per diem role offers flexibility, autonomy, and the opportunity to build meaningful patient relationships while delivering high-quality care in the community. As a Per Diem LPN, you will provide skilled nursing care to clients in their homes throughout Westchester County, working independently with support from a responsive clinical team. Assignments include 12-hour day shifts, overnight shifts, and skilled nursing visits.
Requirements
- Active New York State LPN license and current BLS certification
- Minimum 1 year of recent LPN experience in an acute care setting
- Eligibility to work in the United States
- Recent physical exam (within 1 year)
- Current PPD or QuantiFERON (within 1 year) or chest X-ray (within 5 years)
- MMR immunizations within the last 10 years
Nice To Haves
- Home care experience preferred
- Valid driver’s license preferred
Responsibilities
- Provide skilled nursing care to clients in their homes throughout Westchester County.
- Work independently while having the full support of a responsive clinical team.
- Complete assignments including 12-hour day shifts, overnight shifts, and skilled nursing visits.
