Requirements

  • Clinical nursing skills including vital signs, medication administration, wound care, and infection control
  • Strong assessment and observation skills to recognize changes in patient condition
  • Effective communication skills with patients, families, physicians, and care team members
  • Time management and organization to manage multiple tasks and patient needs
  • Compassion and empathy when providing patient-centered care
  • Documentation accuracy and comfort using electronic medical records (EMR)
  • Ability to follow care plans and clinical protocols
  • Problem-solving skills and sound clinical judgment
  • Adaptability and independence especially in home or community-based settings
  • Professionalism and reliability in all interactions
  • Strong patient and caregiver education skills

  • Experience in home health or hospice

Responsibilities

  • Engage in one-on-one patient care, forging meaningful connections while focusing on delivering high-quality services in the comfort of their homes.
