About The Position

All Heart Home Care is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our growing home health team in the Murfreesboro, TN area. We are currently hiring for Full-Time (FT), Part-Time (PT), and PRN positions. This role provides the opportunity to deliver high-quality, patient-centered care in patients' homes while working alongside a supportive interdisciplinary healthcare team.

Requirements

• Current Licensed Practical Nurse (LPN) license in the State of Tennessee
• Current CPR certification
• Valid driver's license, reliable transportation, and current auto insurance
• Strong communication, organizational, and clinical skills
• Ability to work independently while effectively managing assigned patient visits

Nice To Haves

• Previous home health experience preferred but not required

Responsibilities

• Provide skilled nursing care in patients' homes under the supervision of a Registered Nurse and in accordance with physician orders
• Monitor patient conditions and report changes to the supervising nurse and healthcare team
• Administer medications, treatments, and routine nursing care as ordered
• Assist with wound care, disease management, and health education
• Educate patients and caregivers on medications, treatments, and home care procedures
• Accurately document patient visits and care provided in accordance with company policies and regulatory guidelines
• Maintain communication with patients, families, physicians, and other members of the care team
• Promote patient comfort, safety, and independence within the home environment
• Travel throughout the assigned service area to provide patient care
