About The Position

Ambercare Homecare is seeking a part-time Licensed Practical Nurse to join its growing team in Albuquerque, New Mexico, serving Albuquerque and surrounding areas. The schedule is Monday through Friday or weekends. The nurse provides direct patient care under the state Nurse Practice Act, implements the plan of care initiated by a registered nurse, and documents care accurately and promptly. The role also includes educating patients and caregivers, coordinating home health services, using equipment and supplies safely and efficiently, and supporting professional development.

Requirements

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years of nursing experience.
• Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
• Possess and maintain valid CPR certification.
• Reliable transportation, a current driver's license, and appropriate automobile insurance.

Nice To Haves

• Community or home health experience is preferred.
• Medical/surgical experience is preferred.

Responsibilities

• Provide direct patient care as defined in the state Nurse Practice Act.
• Implement the plan of care initiated by the registered nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Assess patients and educate patients and family or caregivers about self-care techniques and information relevant to the diagnosis and plan of care.
• Participate in coordination of home health services and report identified needs for other disciplines to the registered nurse and/or Clinical Supervisor.
• Use and prepare equipment and supplies effectively and efficiently while adhering to aseptic technique.
• Participate in personal and professional growth and development.
• Perform other duties as assigned by the registered nurse.
• Provide health care instructions to patients as appropriate to the assessment and plan of care.
