Requirements
- Current New York State LPN license.
- 6+ months of experience in home care/ school
- CPR/BLS certification (American Heart Association or Red Cross).
- Physical exam within the past year.
- QuantiFERON within the past year.
- MMR documentation or titers.
Responsibilities
- Provide direct patient care, including administering medications and treatments.
- Monitor and document patient health status and communicate changes promptly.
- Support clients with activities of daily living while maintaining their comfort and dignity.
- Provide 1:1 care for pediatric clients in school settings, ensuring a safe and healthy environment.
- Collaborate with families and the care team to implement personalized care plans.
