About The Position

The Licensed Practical Nurse (LPN) provides nursing care to a select group of residents, including supportive and restorative care, education, and collaboration in implementing resident services plans. The position works under the supervision of a SALSA and, for medication management, under the supervision of the SALSA/RN designee in collaboration with physicians, pharmacists, and outside agencies. This role is at Masonicare at Greenridge Place in Rocky Hill, Connecticut, and is scheduled for 16 hours per week on day and evening shifts.

Requirements

Graduate of an accredited School of Nursing.
Current LPN license.
CPR certification.

Nice To Haves

1–2 years of clinical experience in a hospital or similar health care setting is preferred.

Responsibilities

Collect resident data based on age, needs, and abilities in an ongoing and systematic manner; contribute to resident care and service plans and participate in care planning meetings.
Communicate and collaborate with residents, families, colleagues, and other members of the health care team to meet resident needs.
Educate, supervise, and counsel residents and families regarding assisted-living procedures and other care needs.
Document accurate, pertinent, and comprehensive information in client service records in a timely and concise manner.
Conduct supervisory visits of resident care assistants, companions, and homemakers as required.
Participate in educational programs, in-services, meetings, and workshops, and use current literature to enhance professional skills and knowledge.
Oversee medication management, including ordering, storing, reconciling, and preparing medications for administration and/or CNA prompting, under the supervision of the SALSA/RN designee and in collaboration with physicians, pharmacists, and outside agencies.
Perform other duties as assigned.
