About The Position

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide high-quality care to geriatric residents in a long-term care setting. This role involves delivering direct patient care, administering medications, performing treatments, and supporting the interdisciplinary healthcare team to ensure a safe and effective care environment. The LPN may also take on leadership responsibilities, including supervising Certified Nursing Assistants (CNAs) and assisting with unit operations under the direction of a Registered Nurse (RN) or physician.

Requirements

  • Active Licensed Practical Nurse (LPN) license (State of Arizona or compact license)

Responsibilities

  • Administer medications in accordance with physician orders and scope of practice
  • Perform clinical treatments such as wound care, catheter care, and other nursing procedures
  • Monitor, assess, and report changes in patient condition and response to treatment
  • Assist in the development and implementation of individualized care plans
  • Conduct admission assessments and maintain accurate and timely documentation
  • Provide direct patient care, including assistance with Activities of Daily Living (ADLs)
  • Communicate effectively with patients, families, and interdisciplinary care teams
  • Participate in shift reports and care coordination activities
  • Ensure availability and proper use of medical supplies and equipment
  • Supervise and support CNAs and other support staff as needed
  • Participate in infection control and quality improvement initiatives
  • Attend staff meetings, training sessions, and continuing education programs
