About The Position

PrimeCare Medical, Inc. is hiring Licensed Practical Nurses to work in the medical department at Kent County Detention Center in Chestertown, Maryland. The role provides comprehensive nursing care and performs duties under the supervision of an RN. The position includes a two-week orientation period involving on-the-job training and shadowing with nursing staff. Available schedules include part-time days, PRN second shift, and a full-time MAT schedule Monday through Thursday from 9 a.m. to 1 p.m. Flexibility is required; circumstances may require additional hours, overnight travel, or temporary relocation to meet facility needs.

Requirements

Licensed as a Practical Nurse in Maryland and licensed to practice nursing in the state where the facility is located.
Ability to communicate effectively.
Ability to function under supervision.
Demonstrated clinical competency.

Responsibilities

Provide nursing care for the Medication Assisted Treatment (MAT) Program in accordance with PrimeCare policies and procedures, nursing standards, and scope-of-practice limitations.
Coordinate care for patients receiving medication for substance use disorder, including daily MAT medication dosing according to facility procedures and dosing policy.
Perform duties under RN supervision and protocols established by the Medical Director.
Collaborate with PrimeCare nurses, providers, security, administration, facility staff, and community providers.
Act as a MAT patient liaison through permitted communications and coordinate care by obtaining records, making referrals, and assisting with community reentry and connections to community MAT providers.
Communicate relevant patient and unit information to appropriate personnel and maintain professional relationships with facility, unit, provider, and corporate staff.
Maintain and update electronic medical records, protect medical-record security and privacy, and document and communicate in accordance with HIPAA and 42 CFR confidentiality guidelines.
Ensure compliance with PrimeCare and facility policies, procedures, and established standards.
Monitor nursing care to avoid inappropriate or duplicated services and advise the HSA and charge nurse when issues arise.
Screen for appropriate MAT referrals to providers and ensure follow-up tasks are created.
Comply with pharmacy regulations for ordering, storing, and administering medications for opioid use disorder and alcohol use disorder.
Monitor supplies and equipment inventories and alert the head nurse when items are needed.
Obtain lab specimens as required by MAT program policy.
Attend weekly MAT interdisciplinary team meetings, record minutes, report required data, participate in program evaluation, and maintain records for quarterly and yearly statistics.
Attend seminars, workshops, and conferences at PrimeCare's direction.
In the charge nurse's absence, be operationally responsible and accountable for nursing functions of the medical unit.
Perform other duties as assigned by the charge nurse, HSA, or corporate staff.
Maintain accurate records of controlled substances, narcotics, and sharps in accordance with state and federal laws.
When necessary and permitted by state regulations, conduct sick call and treat selected minor illnesses and injuries under written protocol; refer other cases or those not responding to treatment to an appropriate PrimeCare physician.
Give and receive inter-shift reports to support continuity of care and communicate pertinent information to the oncoming shift.
Observe, report, and document patient symptoms, reactions, and progress, and participate in formulating MAT care plans.
