About The Position

The Licensed Practical Nurse (LPN) for outpatient/Primary Care is responsible for delivering nursing care to adults, youth, and adolescents with severe and persistent mental illnesses within the community-based service area. The LPN functions as a member of the multidisciplinary team, providing individualized nursing care to clients with severe and persistent mental illness and their families. This role also involves acting as a liaison with other MHS service areas and community healthcare providers for the ongoing care of SMD and Residential patients.

Requirements

  • High School Diploma or GED
  • Associates Degree in Nursing
  • 1 year nursing experience preferred
  • Active OH LPN License
  • NVCI
  • CPR (BLS)

Responsibilities

  • Assesses the biological, psychological, socio-cultural, and spiritual aspects of the SMD patient and their family system as appropriate.
  • Analyzes and interprets data, recognizes major and minor defining characteristics, and identifies nursing care needs based on patterns recognized by NANDA.
  • Maintains accurate, objective, and timely documentation in the patient's chart in accordance with MHS policies and procedures.
  • Assists the physician during medication clinic by performing an abbreviated nursing assessment, including vital signs, and documenting findings in the patient record.
  • Screens for pain and encourages patients to follow up with a healthcare provider of their choice.
  • Administers and monitors the use of prescribed medication in accordance with service area policies and procedures, including medication administration, education, monitoring lab values, assisting/supervising patients with self-administration of medication, and coordinating central pharmacy orders.
  • Monitors patient medication and assesses for side effects, completing documentation as needed.
  • Updates medication orders in the computer program to ensure continuity of care.
  • Provides appropriate teaching and learning interventions for patients, families, and significant others affected by severe mental illness.
  • Maintains current knowledge of psychiatric nursing for SMD and Residential patients and independently seeks to expand knowledge based on identified learning needs.
  • Provides nursing services to Residential families (Bridge House, Community Homes, and the Apartment program), including assisting residents with weekly medication boxes, updating emergency information, providing health education groups, and acting as a liaison with primary care physicians, psychiatrists, case managers, and other healthcare providers.
  • Provides education classes for Residential Medication Assistants and Home Operators and conducts in-service trainings as required.
  • Works with extended care facilities and Springfield Regional Hospital to coordinate psychiatric care in conjunction with case management services and psychiatrists.
  • Adheres to all MHS program-wide, MHS nursing service, and BHR service area policies and procedures.
  • Collaborates with patients, families, and significant others throughout the treatment process, focusing on strengths and enhancing skills for promoting wellness.
  • Works in conjunction with the Clark County Jail psychiatrist to provide medication continuity for incarcerated patients.
  • Follows all universal precautions for safety and infection control, and adheres to all state, federal, and Joint Commission health and safety standards.
  • Treats patients and families with dignity and respect at all times, and maintains the strictest confidence of all patient information.
  • Adheres to professional standards, policies, and procedures, federal, state, and local requirements, and Joint Commission standards, including National Patient Safety Goals.
  • Presents a positive image of MHS to other community agencies, caregivers, and citizens.
  • Completes all MHS required education and training, including initial agency orientation, mandatory trainings and educations, and upkeep of all required certifications and licensure as required by state, federal, and regulatory requirements.
