Licensed Practical Nurse (LPN) - Full-Time, M-F (3 Shift)
About The Position
Conexiones ATS/CSS is a 32-bed facility providing clinical stabilization services to adults struggling with substance abuse, including heroin, prescription drugs, and alcohol. The program offers acute treatment services for medically managed withdrawal and residential substance abuse treatment in a safe environment. Casa Esperanza Conexiones, located at Tewksbury Hospital, also provides inpatient medically managed detoxification services, including for individuals with co-occurring mental health issues. The highly structured environment focuses on addiction resolution, building a foundation for sustainable recovery, and developing a healthy lifestyle. The program is staffed by skilled professionals who create individualized treatment plans and determine appropriate aftercare. The Licensed Practical Nurse will implement doctor's orders, monitor medications, record vital signs, maintain medical/nursing records, observe patients, document treatment progress, and coordinate with Case Managers regarding client progress and treatment.
Requirements
- Massachusetts Licensed Practical Nurse
- Nursing Experience: 2 years, required
- Medication monitoring: 1 year, required
- EHR systems: 2 years, required
- Ensure patient confidentiality in accordance with all federal, state, and local laws and regulations
- Knowledge on best practices associated with the American Society of Addiction Medicine
- Knowledge on Medication Assisted Treatment for substance use disorders
- Knowledge of Substance Use Disorder and other co-occurring conditions
Responsibilities
- Complete nursing assessments, COWS, and CIWA assessments upon admission
- Complete COWA/CIWA and ASAM assessments aligned with medical orders for protocols associated to withdrawal management
- Complete medical clearance exam at time of intake
- Complete medication monitoring and all required documentation
- Complete medication counts at the start and finish of the shift of all control and non-control substances
- Work closely with the clinical team during MDT meetings to offer and provide client medical insight as part of case presentations
- Consult with staff and attend case conferences, as needed
- Work collaboratively with the medical director for the medication reconciliation process
- Works to achieve standing orders and medical orders for medication
- Able to organize and keep record of frequency and urgent medical exams
- Directly works in collaboration with community providers for medication refills and reconciliation
- Complete evaluations based on client request for medical attention, emergency and non-emergency triage and physical/mental health rounds for safety and condition monitoring
- Keep electronic health records up to date including progress notes and assessments of service
- Educating clients on their current medical diagnosis, needs and medical histories; educating clients on medication information and usages
- Alerting physicians when conditions worsen or if abnormalities arise
- Works in collaboration with counselors to ensure proper medication management and medication planning
- Works directly with the program director on any medication errors and medication monitoring procedure
- Works uniformly with the recovery specialist on the medication monitoring procedure
- Ensure responsibilities regarding Duty to Warn and Mandated Reporting are fulfilled
