About The Position

Under direct supervision of the registered nurse provides direct patient care to assigned patients. This role requires knowledge of nursing theory and practice acquired through an accredited school of nursing. The Licensed Practical Nurse will assist in gathering data to assess the physical, psychological, social, and spiritual needs of patients. They will collaborate with the registered nurse and interdisciplinary team members in planning for patient needs upon admission, review care plans, and provide input into updating and revising them. The role supports direct patient care utilizing clinical nursing skills and judgment, provides education and reinforcement for patients and families, and initiates appropriate emergency measures when necessary. Responsibilities also include preparing patients for tests and procedures, administering and documenting medications, delegating care to non-licensed personnel, reporting to the Registered Nurse, and performing other duties as assigned. Requirements

  • Knowledge of nursing theory and practice acquired through an accredited school of nursing.
  • Current registration with the Maryland State Board of Examiners of Nurses as a Licensed Practical Nurse.
  • Healthcare Provider (CPR Certification) – not required for Hospice employee.
  • Interpersonal skills, communication (verbal and written) are required in order to instruct and comfort patients and their families and maintain effective contacts with a variety of hospital personnel and maintain appropriate/adequate documentation.
  • Ability to perform and plan professional nursing care resolving complex patient care problems and dealing with patients and their families often during stressful situations.
  • Ability to stand and walk almost constantly (up to 90% of work time) and occasionally (up to 50% of work time) lifting and positioning patients.

Responsibilities

  • Assists in gathering data to assess the physical, psychological, social, spiritual needs of patients.

  • Prepares patients for tests and procedures.
  • Administers and documents medications according to policy and procedure.
  • Can appropriately delegate care to non-licensed personnel and report to the Registered nurse.
  • Report to the Registered nurse with a thorough concise report on patient condition changes or evaluation of interventions.
  • Perform other duties as assigned
