About The Position

Pine Creek Rehab in Salt Lake City, Utah, is hiring a Licensed Practical Nurse to care for residents in a long-term care setting. The role includes ordering prescribed medications, equipment, and supplies; preparing and administering ordered treatments and medications; monitoring residents; communicating changes in residents’ conditions to physicians and/or family members; and ensuring care aligns with each resident’s care plan and wishes. Nurses work 12-hour shifts. Full-time nurses work three days per week; part-time nurses work one to two days per week; PRN nurses work as needed or available. The posting describes a full-time weekend night position, with night shifts from 6 PM to 6 AM.

Pine Creek Rehab offers a Nursing Tuition Reimbursement program of up to $500 per month (pre-tax) for eligible employees attending nursing school or reimbursement for a nursing program. Eligibility requires working part-time or full-time and completing the first 90 days of employment; no contract is required. Listed employee benefits are available only to full-time employees.

Requirements

• Hold a current Utah-issued LPN license in good standing.
• Communicate effectively.
• Be willing to learn local, state, and federal laws and regulations regarding long-term care.

Responsibilities

• Order medications, equipment, and supplies as prescribed.
• Prepare and administer treatments and medications as ordered by the physician.
• Notify the resident’s physician and/or family members when the resident’s condition changes.
• Provide professional nursing services, including tube feedings, suction, catheterization, and changing dressings, packs, and irrigations as necessary.
• Continuously observe and monitor seriously ill residents.
• Advocate for patients and ensure other healthcare team members provide care according to residents’ care plans and personal wishes.
• Complete required charting accurately and in a timely manner.
