About The Position

The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in assisted living, under the supervision of a registered nurse, advanced practice provider, or physician. The LPN demonstrates knowledge of nursing principles for the resident in the relevant clinical setting and practices within the full scope of nursing expertise/knowledge. They utilize appropriate age and population-specific standards related to the physical and psychosocial needs of the resident as per the care plan. The LPN must be able to perform testing to evaluate data reflective of the resident's status and provide the necessary care. They demonstrate the ability to recognize problems, recommend solutions, and respond effectively in emergent situations, ensuring the health, comfort, and safety of residents. The LPN contributes to resident assessments, administers medications and treatments as ordered, and provides technical support to healthcare professionals. They document resident interactions and outcomes related to care, medication, behavior, activity, and diet. The role requires establishing and maintaining effective working relationships with residents, healthcare providers, and the public, and working effectively as part of an interdisciplinary care team. The LPN must communicate clearly and maintain confidentiality. They counsel residents, family members, and/or resident advocates on prevention and treatment plans under the direction of a registered nurse, advanced practice provider, or physician, and possess knowledge of resident education principles to develop self-care programs. Other related duties are assigned as needed.

Requirements

  • Graduated from an accredited school or college of nursing program leading to licensure as a Licensed Practical Nurse.
  • Current licensure as a practical nurse in the state/states of practice.

Responsibilities

  • Works effectively with professional and supportive personnel as a member of an interdisciplinary care team.
  • Communicates clearly and maintains confidentiality of sensitive information.
  • Counsels residents, family members and/or resident advocate, under the direction of a registered nurse, advanced practice provider or physician on prevention and treatment plans.
  • Develops a self-care program using resident education principles.
  • Performs other related duties as assigned.
