About The Position

Tradition is an independent and assisted living community in Brooklyn Park, Minnesota, and a Transforming Age community. Licensed Practical Nurses provide care, compassion, and treatment to residents to help meet their comprehensive needs. The listed pay range is $28.00 to $34.00 per hour, depending on experience (DOE).

Requirements

• Graduate of an accredited LPN program.
• Active LPN license in the state, in good standing.
• Experience in a nursing home; assisted living or long-term care experience is preferred but not required.
• Flexibility to work occasional weekends or holidays as needed.
• Knowledge of DHS regulations.
• Excellent interpersonal and communication skills, and the ability to work collaboratively with team members and external individuals.
• Completion of a background check and drug screen; other pre-employment screenings may include a physical, TB test, and fingerprinting per state or local guidelines.

Responsibilities

• Provide compassionate and effective care and treatments to residents.
• Work with a team of professionals to ensure residents’ comprehensive needs are met.
• Review routines, the calendar of events, and resident needs.
• Ensure medications and supplies are ordered, in stock, and counted.
• Ensure charting and record keeping are accurate and complete.
• Attend meetings and trainings as required.
• Promote Transforming Age’s Mission, Vision, and Values, and its Culture of Excellence, in all areas of responsibility.
• Perform other duties as assigned.
