DDS Uxbridge House - Licensed Practical Nurse (20 Hours)
About The Position
The Licensed Practical Nurse works in close collaboration with the Registered Nurse to ensure the medical needs of people supported are met. This position involves acting as a liaison between the program and other stakeholders, including family members and other medical providers. This role is not in an office or hospital setting and offers the ability to make connections with patients in an innovative and person-centered domain. The pace is slower than a hospital setting, allowing for work with people on a long-term basis, providing a unique opportunity for those looking for a clinically based, supportive, and nurturing role.
Requirements
- High School Diploma, GED or equivalent, required.
- Certified Licensed Practical Nurse in Massachusetts, required.
- Excellent assessment, documentation, and verbal communication skills, required.
- Valid driver’s license and acceptable driving history, required.
Nice To Haves
- Experience with adults with disabilities, preferred.
Responsibilities
- Oversee medication ordering, counts and refills following physicians written orders.
- Monitor individual’s medical status and transport, or delegate transportation to and from medical appointments.
- Advocate for individuals with physicians and other service providers.
- In collaboration with the staff team, ensure health and wellness standards are being implemented including development of nutritious meals.
- Attend all Individualized Support Plan (ISP) meetings and complete necessary documentation for the ISP’s.
