Reporting to the Director of Care or Director of Health & Wellness, the Licensed Practical Nurse (LPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care. This role involves working as a member of an inter-professional team, applying a person-centered approach to care. The LPN will complete and ensure resident's clinical care assessments, care/service plans, and documentation are accurate and up-to-date, communicating changes as required. They will also implement and evaluate the effectiveness of care/service plans, assist with scheduling and participating in resident's care conferences, and facilitate resident move-ins, move-outs, and transfers. Direct resident care, medication/treatment administration, and documentation are key components, adhering to regulatory and Extendicare standards. The LPN will process orders, safely keep drugs and narcotics, and administer them in compliance with all regulatory requirements and Extendicare policies. They will also facilitate resident/family education, coordinate and evaluate resident care plans with various stakeholders, and coordinate nursing team assignments for efficient service delivery. Responding to significant changes in resident condition or medical emergencies, providing first aid, and teaching/delegating to unregulated care providers are also responsibilities. The LPN will coordinate emergency response in the absence of leadership, respond to resident/family concerns, and follow/assist in the coordination of Infection Prevention and Control, Health and Safety, and Clinical Programs.
