BAYADA Home Health is currently seeking an experienced Licensed Practical Nurse, for a full-time opportunity performing home health visits with adult and geriatric clients in territories throughout Atlantic County, NJ.

Requirements

• Current Licensed Practical Nurse (LPN) license in NJ
• Minimum one year of nursing experience required
• Strong clinical assessment and communication skills

Nice To Haves

• Previous home health experience preferred, but not required

Responsibilities

• Follow established care plans developed by the Registered Nurse (RN)
• Perform skilled nursing visits in patients' homes
• Assess patients and monitor their condition
• Administer prescribed medications and treatments
• Provide wound care and disease management education
• Reinforce patient and caregiver teaching
• Accurately document patient care and clinical observations
• Communicate changes in patient condition to the supervising RN and physician
• Collaborate with an interdisciplinary team to ensure optimal patient outcomes
