About The Position

The Licensed Practical Nurse provides nursing care to patients at Rehabilitation Hospital of Oak Creek, a new 40-bed inpatient rehabilitation facility. The role supports comprehensive physical medicine and rehabilitation programs aimed at maximizing patients’ health, function, and quality of life. The nurse assesses patients, identifies needs, delivers appropriate interventions and age- and condition-specific care, and works with patients and families throughout the rehabilitation stay and before discharge. The nurse also collaborates with the interdisciplinary team and contributes to care planning, quality improvement, and timely clinical documentation.

Requirements

Active Wisconsin or compact-state LPN license required. If relocating, licensure must be updated to the new Primary State of Residence before employment begins.
Associate's degree required.
AHA Basic Life Support (BLS) certification required within 30 days of hire.
Critical thinking skills, decisive judgment, and ability to work with minimal supervision.
Ability to work in a stressful environment and take appropriate action.

Responsibilities

Accurately perform patient assessments and identify patient needs.
Identify and initiate appropriate nursing interventions.
Provide patient and family teaching throughout the rehabilitation stay and prior to discharge.
Provide care appropriate to the patient’s condition and age.
Work cooperatively and actively participate with other members of the interdisciplinary team.
Participate in Continuous Quality and Performance Improvement (CQPI).
Complete timely and appropriate documentation relating to medical necessity in the medical record.
Complete and revise the Interdisciplinary Care Plan for each patient.
Perform timely and accurate QI assessments.
