About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under a Registered Nurse’s supervision to provide skilled nursing care to patients in their homes. The role includes coordinating care with the RN and physician, educating patients and caregivers, completing nursing procedures, and documenting care accurately and promptly. The position offers development opportunities and compensation based on a combination of per-visit-point pay for visits and hourly pay for certain non-visit activities. The stated annual total cash target assumes full-time employment (40 weekly hours) at full productivity. Compensation varies based on factors including local labor markets, education, work experience, productivity, and performance. Benefits and incentives are subject to eligibility requirements. Candidates must pass a drug test before beginning employment.
Requirements
- At least 1 year of LPN or LVN experience.
- Current LPN license specific to the state or states assigned for work.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and liability insurance.
- Pass a drug test before beginning employment.
Responsibilities
- Verify the plan of care with the RN before each visit.
- Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient’s condition changes.
- Teach patients and caregivers self-care techniques.
- Check vital signs.
- Review medications and perform medication reconciliation at each visit.
- Perform specialized procedures ordered by a physician, such as Foley catheter changes.
- Prepare equipment and materials for treatments using aseptic technique.
- Complete documentation accurately and on time.
