About The Position
Licensed Practical Nurse (LPN) responsible for providing patient care in a fast-paced outpatient setting, including rooming patients, triage, and clinical support under the direction of a provider. Ensures efficient patient flow while delivering high-quality, patient-centered care.
Requirements
- Active, unrestricted LPN license in Vermont
- Current CPR certification
- Strong clinical assessment and triage skills
- Excellent communication and interpersonal skills
- Ability to multitask and thrive in a fast-paced environment
- Patient-focused and team-oriented
- Strong organizational and time-management skills
- Ability to prioritize and adapt quickly
- Professional, dependable, and detail-oriented
Responsibilities
- Room patients and maintain efficient patient flow for scheduled and walk-in visits
- Perform triage (in-person and phone) and prioritize patient needs per protocol
- Obtain and document vital signs, medical history, and clinical information in the EMR
- Communicate patient concerns and clinical findings to providers
- Assist providers with exams, procedures, and patient care
- Administer medications, immunizations, and treatments as ordered
- Process prescription refills and patient calls/messages
- Perform or assist with in-office testing and specimen collection
- Provide patient education and support for care plans and follow-up
- Maintain accurate, timely documentation and uphold confidentiality standards
