About The Position
Riggs Community Health Center is currently seeking a Licensed Practical Nurse (LPN) to provide care to a diverse population in Lafayette, IN, and the surrounding areas. You will work closely with our team of established health care providers, including on-site behavioral health specialists, continually providing a growing variety of services to the community in a small clinic setting. Riggs Community Health Center brings critically needed integrated health services, wellness, and education to the under-served population in our community. Riggs Community Health Center is a highly rated non-profit and Federally Qualified Health Center (FQHC), and Patient-Centered Medical Home (PCMH) certificated clinic, dedicated to the mission of improving access to quality, cost effective, comprehensive health care with respect and compassion to under-served community members. We serve Tippecanoe, Boone County, and Clinton County community members with over 65,000 patient encounters annually and growing! Requirements
- Licensed Practical Nurse (LPN or RN)
- Experience-Based Salary
- Full utilization of EMR (NextGen) and other health care services technologies
- Monday - Friday 8:00am - 5:00pm (40 hours only)
Responsibilities
- Prepares patients for examination: Greets patients, Reviews symptoms, treatment, and past health/medical history. Takes B/P, weight, height, temperature, and other measurements such as peak flow and/or vitals as appropriate to complete health assessment in clinical area. Performs vision, hearing screenings and spirometry. if appropriate.
- Assists with provision of care that includes treatment, teaching and counseling.
- Assists with patient scheduling and encounter forms.
- Answers and triages telephone calls and callbacks as instructed by registered nurse, nurse practitioner and/or physician.
- Cleans and stocks examination rooms and maintains room cleaning schedule.
- Responsible for set-up, assisting and post-cleaning for special procedures.
- Maintains clinical records and logs for clinical area.
- Responsible for maintaining, inventory of medical, office, supplies for clinical area.
- Files charts, lab reports, patient forms and information in the medical records.
- Requests medical records from other health care providers.
- Assists with chart flow/medical records in (RCHC) and for other providers.
- Assists with the planning and implementation of programs for patients and families to increase awareness of relevant medical conditions, decrease risk-behaviors, and increase responsibility for self-care.
- Attends and participates in in-service education for staff, quality assurance program, and risk-management programs.
- Relieves and substitutes for other staff in other clinical areas as needed for clinic operations and/or other clerical staff as needed.
