About The Position

Ridgewood Terrace, a Concord Health Systems facility, is seeking a dependable and compassionate Licensed Practical Nurse (LPN) to join their team. This is a role within a locally owned and operated nursing home group serving Western Kentucky, with a focus on providing respectful, attentive care that supports the dignity, comfort, and well-being of each resident. The LPN plays a key role in coordinating resident care, administering treatments, and ensuring a calm, supportive clinical environment during the shift.

Requirements

Current Licensed Practical Nurse (LPN) license in the state of Kentucky
Reliable attendance and ability to work assigned shifts (including nights if applicable)
Commitment to professional, compassionate resident care

Nice To Haves

Experience in long-term care, skilled nursing, or nursing home environment
Strong clinical assessment and critical thinking skills

Responsibilities

Administer medications and treatments in accordance with physician orders and facility policy
Assess resident conditions and respond to changes in health status
Provide direct nursing care in a skilled nursing and long-term care setting
Accurately document care and treatments in the medical record system (EMAR/EHR)
Communicate effectively with CNAs, providers, and interdisciplinary team members
Ensure compliance with state and federal healthcare regulations
Support and supervise nursing assistants as needed during the shift
Contribute to a respectful, resident-centered environment
