About The Position

The Licensed Practical Nurse (LPN) will provide skilled nursing care in the Intermediate/Medical Nursing Home Care Unit and Psychiatric units across both the Montrose, NY and Wappingers Falls, NY campuses of the VA Hudson Valley Healthcare System (VAHVHCS). This role involves administering and documenting medications, monitoring patient responses and condition changes, and collaborating with various healthcare professionals. The LPN will operate independently and under Nurse Manager supervision, and must complete all VA mandatory training prior to starting.

Requirements

Active, current LPN licensure in any U.S. state, D.C., Puerto Rico, or U.S. territory; must be in good standing with no restrictions
CPR/BLS - American Heart Association (required)
Graduate of a state-approved school of practical or vocational nursing
Competence caring for adult psychiatric, medical, and geriatric patients
Ability to prepare, administer, and document oral, topical, subcutaneous, intramuscular, and/or IV medications
English fluency required (speaking, reading, writing)

Nice To Haves

Proficiency with VA electronic medical records (CPRS/VISTA) preferred

Responsibilities

Provide skilled nursing care in the Intermediate/Medical Nursing Home Care Unit and Psychiatric units across both campuses
Administer and document medications; report patient responses and condition changes to RN or physician
Collaborate with healthcare professionals across ambulatory care, critical care, nursing home, and ED units at both Montrose and Castle Point VAMCs
Operate independently and under Nurse Manager supervision
Complete all VA mandatory TMS training (VA Privacy & Information Security, Mandatory Transient Staff Training) prior to start
