About The Position

The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a Long-Term Care (LTC) setting. This role works collaboratively with Registered Nurses, physicians, and interdisciplinary care team members to deliver safe, high-quality, and resident-centered care in compliance with organizational policies, state regulations, and safety standards.

Requirements

  • Minimum of 2 years of Long-Term Care (LTC) and/or Rehabilitation experience required
  • Active Licensed Practical Nurse (LPN) license (state-specific and in good standing)
  • Current BLS/CPR certification
  • Strong clinical judgment, communication, and documentation skills
  • Ability to work a full 12-hour day shift and meet contract requirements

Responsibilities

  • Administer medications and prescribed treatments in accordance with physician orders
  • Monitor, assess, and document resident conditions and responses to care
  • Assist residents with activities of daily living (ADLs)
  • Collaborate with Registered Nurses, physicians, and interdisciplinary care team members
  • Identify and promptly respond to changes in resident condition and report appropriately
  • Follow individualized care plans and established protocols
  • Maintain compliance with state regulations, safety standards, and organizational policies
