We are seeking a dynamic and experienced individual for the role of Home Health and Hospice Licensed Practical Nurse, LPN, who shares our desire to make healthcare better for everyone. Join our Aegis Family as a Home Health and Hospice Licensed Practical Nurse (LPN) where you will play a crucial role ensuring our patients receive exceptional care. Our LPN's are responsible for verifying and executing the plan of care as prescribed by physicians and under the guidance of a Registered Nurse. This includes providing essential nursing treatments, promptly addressing patient pain, assisting with activities of daily living, and educating both patients and caregivers on self-care techniques. They also handle medication administration, ensure the availability of necessary supplies, maintain accurate patient records, and consistently liaise with supervising RNs or physicians on patient assessments and any changes in conditions. Moreover, they actively contribute to team meetings to ensure cohesive patient care coordination.

Requirements

  • Current Licensed Practical Nursing License in state of practice
  • Ability to use a full range of motion
  • Ability to lift 35lbs
  • Effective verbal & written communication skills
  • Effective time management skills
  • Valid Driver's License (clean record)
  • Reliable transportation
  • Proof of current auto insurance & registration
  • Valid AZ Fingerprint Clearance Card
  • Current CPR Certification
  • Negative TB Test

Responsibilities
