About The Position

Assists Case Managers in the initial interview of enriched housing resident in conjunction with the case manager to determine appropriateness for residency at Cedarbrook based on admission standards and regulations. Works in conjunction with the case manager to identify and continually evaluate each enriched housing resident’s needs, interests, strengths, and determine the capability of Cedarbrook Village, Inc., to meet those needs and then document this information on the Individual Service Plan (ISP). Assures updates and completion of the ISP and care plan as per the review schedule and as needed. Performs assigned nursing procedures according to the regulations and under the direction of the RN for the comfort and well-being of the residents. Responsible for the timely and accurate administration and documentation of physician ordered medication. Follows facility procedure for the timely ordering and receiving of physician prescribed medication. Transcribes physician orders according to facility policy and procedure and assures proper interpretation and execution of same. Monitors for changes in residents’ conditions with appropriate chart documentation and refers problem areas to physician, and RN. Also communicates changes in resident conditions to family representative as appropriate. Establishes work assignment for the HHA’s and supervises same to assure thorough task completion. Assists the residents as needed with bathing, dressing or grooming services. Assures that discontinued/expired medications are disposed of according to facility policy. Assists with keeping medication carts clean, organized, and maintained according to pharmacy recommendations. Performs quality assurance audits as directed. Maintains documentation of shift occurrences to promote thorough reporting of resident activity to oncoming shift. Notifies RN/ Supervising Nurse of Resident needs including but not limited to: wound care, dressings, treatments, and skin concerns/healed areas. Notifies RN of any concerns/changes in care needs expressed or noted by any Resident. Works under limited supervision. Assists in the training of new staff on their assigned shift. Performs duties according to the highest ethical and moral standards and in adherence with the facility’s corporate compliance plan and all applicable governing laws. Adheres to the facility’s confidentiality statement in accessing and safeguarding protected health information (PHI) of residents and employees, as necessary to perform job duties. Completes second check of all physician/provider orders to ensure accuracy. Prints daily and reviews all Electronic Medical Record reports as assigned by the RN and/or VP of Assisted Living Services. Reports include Orders: Medication Admin Audit Report to verify no missed medications and review of “code 13: Pending Delivery” Medications needing follow-up; POC: Audit Report (Care Services- Point of Care) reviewing any missing or inaccurate document and report findings each day to RN and/or VP of Assisted Living Services. Review daily Residents on wellness checks in Electronic Medical Record and ensure frequency and stop dates are completed as per the Case Manager/ISP. Performs any other duties as assigned by RN or Vice President of Assisted Living Services.

Requirements

  • Graduate of an approved school for licensed practical nursing.
  • Currently licensed in New York State to practice nursing as an LPN.
  • Leadership ability.
  • Effective communication skills.
