About The Position
Family Health and Rehab is a distinguished Skilled Nursing Facility dedicated to providing residents with a comfortable, home-like environment that promotes independence. We are seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate healthcare team. The ideal candidate will possess strong clinical skills, a passion for patient care, and experience working in diverse healthcare settings. As an LPN, you will be responsible for delivering high-quality nursing care, supporting patient assessments, and ensuring compliance with healthcare standards within our long-term care and rehabilitation environment.
Requirements
- Valid LPN license
- Current CPR/BLS certification
Nice To Haves
- Prior experience working in various healthcare environments such as long-term care facilities, hospitals, or assisted living communities.
- Experience with critical care settings like ICU or Level I Trauma Centers.
- Familiarity with medical coding systems including ICD-9 and ICD-10.
- Proficiency in EMR/EHR systems such as Point Click Care.
- Experience in hospice care, dementia or memory care, pain management, infection control practices, and working with individuals with developmental disabilities or behavioral health needs.
Responsibilities
- Provide direct patient care by monitoring vital signs, administering medications, and performing routine assessments.
- Assist with medical procedures such as IV infusions, catheterizations, and sterile techniques while adhering to aseptic protocols.
- Document patient information accurately using Electronic Health Record (EHR) systems such as Point Click Care.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans focused on patient needs.
- Support patients with activities of daily living, including mobility assistance and nutrition management.
- Conduct triage assessments to prioritize patient needs in urgent or emergent situations.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient records and medical information.
