About The Position

American Senior Communities is hiring a Licensed Practical Nurse (LPN) at Heritage House. In this role, you will build meaningful relationships by learning about residents’ and staff members’ life stories. You will use your nursing skills to promote residents’ physical and psychosocial well-being and help provide coordinated, comprehensive care in a supportive team environment. Requirements

Hold a current and valid Licensed Practical Nurse license in the state of Indiana.
Have proficient medication management skills.
Be able to conduct thorough assessments and accurately document changes in resident condition.
Have a strong passion for geriatric nursing and commitment to senior care excellence.
Have excellent communication and interpersonal skills.
Demonstrate the C.A.R.E. values: Compassion, Accountability, Relationships, and Excellence.
Work collaboratively and toward the common goal of excellent resident care.

Responsibilities

Create a comforting and engaging atmosphere for residents.
Promote teamwork within the care team to meet residents’ needs.
Collaborate with nursing and other facility teams to ensure coordinated, comprehensive care.
Use nursing skills to promote residents’ physical and psychosocial well-being.
Conduct thorough resident assessments and accurately document changes in condition.
