About The Position

Adoration Home Health and Hospice is seeking a passionate, dedicated Home Health LPN to join our team in Mebane, NC. Our Home Health LPNs provide expert, patient-centered care. The Home Health LPN provides 1:1 care for clients following the client's plan of care at all times while maintaining client's dignity and independence. This role involves administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results. Accurate documentation of nursing actions, communication with family, case manager, and physicians, and documenting progress and outcomes for established goals are key. The LPN will also inform the physician, case manager, and families of changes in the client's medical condition and needs, and coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are always on hand. Ensuring all patient needs are being met in accordance with the instructions of the physician is also a core responsibility.

Requirements

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification

Nice To Haves

• 1+ years of nursing experience preferred
• Home Health care experience preferred

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence

• Ensuring all patient needs are being met in accordance with the instructions of the physician
